Notice of Privacy Practices

ProCare Occupational Health is committed to protecting the privacy and security of your health information. This Notice describes how medical information about you may be used and disclosed, how you can access this information, and your rights as a patient. Please review it carefully.

ProCare Occupational Health, located at 805 S Auburn St, Kennewick, WA 99336, is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We are required by law to: maintain the privacy and security of your protected health information (PHI); provide you with this Notice of our legal duties and privacy practices; follow the terms of the Notice currently in effect; and notify you in the event of a breach of your unsecured protected health information.

How We May Use and Disclose Your Health Information

For Treatment

We may use your health information to provide, coordinate, or manage your medical care. In the context of occupational medicine, this may include sharing relevant medical findings with an employer-designated physician reviewing a fitness-for-duty determination, as permitted by applicable law.

For Payment

We may use or disclose your health information to obtain payment for services we provide, including submitting claims to your health insurer, workers' compensation insurer, or your employer's occupational health account.

For Healthcare Operations

We may use or disclose your health information in connection with our healthcare operations, including quality assessment, staff training, and business planning.

Employer-Directed Services

If your employer has arranged for ProCare to provide occupational health services — such as pre-employment physicals, drug testing, DOT physicals, or workplace injury care — we may be permitted or required to disclose certain information to your employer as allowed under HIPAA (45 CFR §164.512(b)). We will inform you at the time of service when your employer will receive results.

Workers' Compensation

We may disclose your health information as authorized by and to the extent necessary to comply with Washington State workers' compensation laws, including disclosures to the Washington State Department of Labor & Industries (L&I), your employer, and your workers' compensation insurer.

Legal Requirements and Public Health

We will disclose your health information when required to do so by federal, state, or local law; for public health activities; to health oversight agencies; in judicial or administrative proceedings; or to law enforcement as required or permitted by law.

Serious Threats to Health or Safety

We may use or disclose your health information when necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.

Uses and Disclosures Requiring Your Authorization

Other uses and disclosures of your health information not described above will be made only with your written authorization. You have the right to revoke that authorization at any time in writing. Uses and disclosures that always require your written authorization include: most uses of psychotherapy notes; uses for marketing purposes; and the sale of your health information.

How to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with ProCare Occupational Health at (509) 221-1039 / info@procareforyou.com, or with the HHS Office for Civil Rights at hhs.gov/ocr/privacy/hipaa/complaints / 1-800-368-1019. You will not be penalized or retaliated against for filing a complaint.

Digital Information Collection

When you visit procareforyou.com, we may collect non-personally identifiable information for analytics purposes. When you submit the employer inquiry form, we collect the business contact information you provide. This information is used solely to respond to your inquiry and is not protected health information.

Cookies and Site Traffic

This website uses cookies to support basic site functionality and, where analytics are enabled, to understand site usage patterns. Cookies do not collect or store protected health information. You may disable cookies in your browser settings at any time.

Secure Online Scheduling

Patient appointment scheduling is managed through DrChrono, a HIPAA-compliant electronic health records and practice management platform. Health information you provide during the booking process is transmitted securely to DrChrono and governed by DrChrono's privacy and security practices. ProCare Occupational Health has a signed Business Associate Agreement with DrChrono as required by HIPAA.

Third-Party Partners

Where any third-party provider may access, process, or store protected health information on our behalf, ProCare has executed a Business Associate Agreement with that provider as required by HIPAA.

Children's Privacy

This website is not directed to children under the age of 13. We do not knowingly collect personal information from children under 13. If you believe your child has provided personal information through our website, please contact us at info@procareforyou.com.
