At Your Appointment

  • Medical history and current health review

  • Head-to-toe physical examination

  • Vision and hearing screening

  • Blood pressure and vital signs

  • Urinalysis

  • Additional components as required by your employer (ECG, PFT, treadmill, labs, drug screen)

  • Results documented and sent directly to your employer

Before You Arrive

  • List of current medications and dosages

  • Any relevant medical records (prior injuries, surgeries, chronic conditions)

  • Your photo ID

  • Insurance card if applicable

  • Name of referring employer or company 

  • Any forms sent to you in advance by ProCare or your employer
